(1200) A Systematic Review of Laryngeal Sarcoidosis

Introduction: Sarcoidosis is a systemic condition that can involve the head and neck region; less commonly, sarcoidosis can directly involve the larynx. The objective of this study is to review the symptomatology, treatment, and outcomes of laryngeal sarcoidosis.

Methods: A Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA)-guided systematic review was performed utilizing PubMed, Web of Science, Cochrane, and SCOPUS to identify case reports describing sarcoidosis in the larynx. Two reviewers reviewed each article and extracted data. Exclusion criteria consisted of articles that did report sufficient clinical information, reviews, non-English studies, basic science, and animal studies. The following data were extracted: demographics, symptoms, imaging, treatment, and outcomes.

Results: Altogether, 124 articles met inclusion criteria for a total of 146 cases. Mean age at diagnosis was 36 years old (range 6-84), and 64% of patients were female. The most common presenting symptoms were dyspnea, hoarseness, and dysphagia. The most common locations of sarcoid lesions were the epiglottis, supraglottic region, and aryepiglottic folds. Systemic steroids, intralesional steroid injections, and tracheostomies were the most common treatments. Operative management was rare and reserved for refractory cases. The majority of patients had symptomatic improvement following treatment.

Conclusions: Laryngeal sarcoidosis is an uncommon condition that can have an acute and chronic impact on quality of life and airway security. Most patients identified in our review were young/middle-aged females both with and without a history of sarcoidosis. Laryngeal sarcoidosis is a crucial differential to identify in patients with prolonged hoarseness, dyspnea, or dysphagia.
