(1230) Sleep Quality and Dupilumab

Introduction: The introduction of biologic therapy for the treatment of chronic rhinosinusitis with polyposis (CRSwNP) has dramatically changed the management of these patients and greatly improved their quality of life (QoL). The Sinonasal Outcome Test Questionnaire (SNOT-22) is commonly used to monitor the outcome of this therapy in terms of QoL and includes a sub-domain related to sleep disturbance called "Sleep-SNOT" (items 13 to 20). The purpose of this study is to evaluate if Dupilumab therapy improves patients' sleep quality using the Sleep-SNOT.

Methods: A retrospective analysis of patients treated with Dupilumab at University Hospital of Ferrara from August 2021 to June 2023 was performed. We included patients with at least 6 months of follow-up. Data from the Sleep-SNOT and its individual items at 6 and 12 months were compared by Wilcoxon test. Given the small number of patients treated with other biologic therapies, we decided to exclude them in order to have a homogeneous sample.

Results: We enrolled 31 patients, 19 males and 12 females, with a mean age of 58.3 (+- 13.1) years. Before treatment, the patients had a mean NPS of 5.9 (+- 0.9), SNOT-22 of 66.4 (+- 13.2) and Sleep-SNOT of 25.7 (+- 7.5). After 6 months of treatment, all parameters analyzed were statistically significantly reduced (p < 0,01). The mean NPS was 2.3 (+- 2.4), the SNOT22 was 25.9 (+- 22.7), and the Sleep-SNOT was 10.1 (+-10.2). All Sleep-SNOT items decreased in a statistically significant way after 6 months. If we compare the results obtained after 6 months with those obtained after 12 months, there is no statistically significant change. The only item that continues to decrease is the one related to fatigue (Item 17) (p=0,04).

Conclusions: Dupilumab therapy significantly improves the quality of sleep in treated patients.
This important result is achieved after only 6 months of treatment and is maintained in subsequent months. In addition, patient-perceived fatigue tends to improve continuously throughout the first year of therapy.
