PROGRAM DESCRIPTION: This session will review the current techniques for diagnosis and management of pediatric patients with inducible laryngeal obstruction (ILO), also known as paradoxical vocal fold motion (PVFM). The session will be split into an informational panel session and case-based presentation. The goals of this session are to review the basic pathology of ILO, further participants understanding of ILO by introducing them to lesser-known diagnostic and treatment options, and discuss the set up for continuous laryngoscopy during exercise (CLE). Panelists will be composed of multi-institutional pediatric otolaryngologists who regularly treat patients with ILO in their practice.

We will start with a review of the pathophysiology of ILO, then will focus on contemporary topics including behavioral and psychiatric considerations, voice implications, continuous laryngoscopy, and patient-reported outcomes. The discussion on continuous laryngoscopy will include institutional experience of implementation of a clinic specifically for diagnosis of pediatric patients with exercised induced laryngeal obstruction (EILO). It will also touch on different techniques for performing CLE, such as use of commercially available and 3-dimesional printed scope holders. An example model of a 3-D printed scope holder/helmet used for CLE will be available for participants to interact with at the end of the panel.

Learning Objectives:

Familiarize participants with the multi-disciplinary, multi-modality work-up and treatment of pediatric patients with inducible laryngeal obstruction/ paradoxical vocal fold motion.
Illustrate the multidimensional impact of inducible laryngeal obstruction on the pediatric patient, which includes physical, behavioral, social, and quality of life components.
Provide tips on set up and implementation of continuous laryngoscopy for diagnosis and treatment of exercise induced laryngeal obstruction.
