The Effect of Comorbidities on Cochlear Implantation Outcomes in Adults Under the Age of Sixty

Introduction: Comorbidities can negatively affect cochlear implantation (CI) outcomes in elderly patients, but little is known about their effect on younger adults (20-60 years) who are often employed and highly social. This study examines the relationship between comorbidities and CI outcomes in non-elderly adults.

Methods: We retrospectively reviewed electronic medical records for all CI recipients between 20 to 60 years old at an academic medical center from 2015-2019. Patient data was collected including comorbidities, demographics, etiology, and length of deafness (LOD). Patients’ Charlson Comorbidity Index (CCI) was calculated. The primary outcome was speech perception scores at 1-year for words (CNC) and sentences in quiet and noise (AzBio).

Results: A total of 118 patients underwent CI (17% 20-29 years, 22% 30-39, 21% 40-49, 43% 50-60) with an average of 1.8 comorbidities. Mean LOD was 19.9 years, 79% of patients had progressive hearing loss, and the most common etiologies were unknown (52.5%) and neurofibromatosis type 2 (7.6%). Of the cohort, 35% had no comorbidities, and the most frequent comorbidities were hypertension (18%), asthma (12%), anxiety (10%), hyperlipidemia (9%), and acoustic neuroma (9%). Comorbidity frequency was similar across age groups, but cardiovascular comorbidities varied by patient decade (50-60 yrs: 41% vs 20-49 yrs: 8-20%, p = 0.003). Compared to other studies on elderly CI outcomes, our cohort had decreased comorbidity burden with reduced cardiac events and neurological conditions. In addition, there were no differences in 1-year CNC scores or AzBio scores based on number of comorbidities in our study population. According to a generalized linear model of comorbidities, none significantly affected 1-year AzBio scores. Patients’ CCI did not significantly correlate to postop scores when adjusted for age and LOD.

Conclusions: Subjects showed improved speech understanding post-CI. The number and type of comorbidities were not significant predictors of 1-year speech perception scores, suggesting adult non-elderly CI users with comorbidities can expect comparable outcomes to those without.
