Long-Term Effect of Botulinum Toxin as Treatment for Retrograde Cricopharyngeus Dysfunction

Introduction: To report the long-term effect of botulinum toxin (BT) injection into the cricopharyngeus muscle (CPM) for retrograde cricopharyngeus dysfunction (R-CPD) using a Burp Survey Index (BSI) survey.

Methods: Retrospective and prospective studies were performed on R-CPD patients between 2021-2023 treated with BT injection into the CPM and had full responses at the 1-month follow-up. The study assessed R-CPD treatment response at least six months following treatment using a BSI survey. Long-term groups were divided into “6-12 months”, “12-24 months”, and “24-36 months.” BSI score greater than 29 defined as having persistent RCPD symptoms

Results: A total of 27 patients were included in the study. More than half of the patients were female (N = 14, 51.9%), with a median age at diagnosis of 29.8 (range 19-47). In the “6-12 months” group (N = 12), the mean initial BSI score was 39 (SD = 2.6) and the mean follow-up BSI score was 9 (9.1) (p < 0.0005). Only 2 (16.7%) patients had persistent symptoms at follow-up. In the “12-24 months” group (N = 5), the mean initial BSI score was 39.8 (SD = 1.6), and the mean follow-up BSI was 11.8 (2.56). No patient had persistent symptoms. In the “24-36 months” follow-up group (N = 10), the mean initial BSI score was 36.9 (SD = 4) and the mean follow-up BSI was 17.6 (7.5). Only two patients (20%) had persistent symptoms at follow-up.

Conclusions: The majority of patients still have a full response to BT injection and have the ability to belch at least 6 months from treatment.
