An Appraisal of Mandibular Trauma

Introduction: Craniomaxillofacial trauma (CMF trauma) imposes a significant economic burden on institutions. The purpose of this investigation was an assessment of mandibular fracture constellations, insurance payers, costs, and reimbursements.

Methods: A retrospective chart review of mandible fractures at a Level One Trauma Center was conducted from January 2017 to December 2022. Demographics, fractures, insurance, treatment, encounter costs, charges, and reimbursements were tabulated and analyzed using R software.

Results: A total of 233 patients were included in the study. The average age of the patients was 38 years old (range: 18 – 99). 71% were male. 16% were isolated angle fractures. 20%, 31%, 34%, and 15% were found to have commercial, public, self-pay, and other as their insurance type, respectively.

The mean encounter cost of isolated and multiple mandibular fractures differed ($5,652, $11,501; p < 0.001) without difference in mean reimbursements ($8,636, $10,360; p = 0.64). Despite no difference between mean encounter cost for self-pay versus other payers ($6,816, $7,903; p = 0.31), reimbursement differed ($3,176 and $12,277; p < 0.001). In an effort to approximate the cost of surgical repair, the mean operating room (OR) charges were analyzed for single and multiple mandible fractures. A significant difference between single and multiple fractures was observed ($40,956, $55,764; p < 0.001), accounting for 43% and 49% of total charges (p = 0.03).

Conclusions: The cost of mandibular fracture repair is significant. There was not a concordant increase in reimbursement for multiple fractures. Reimbursement also varies with payer type. Implementing programs to promote insurance acquisition, minimizing repair cost, and raising awareness of the economic burden may improve sustainability of mandibular fracture repair.
