Utilization of Synthetic Electrospun Fiber Matrix in Head and Neck Intraoral Reconstruction

Introduction: Head and neck cancer surgery often necessitates functional and aesthetical complex reconstructions. In addition to radiotherapy and comorbidities, the intraoral environment present additional difficulties for wound healing. A recently introduced synthetic electrospun fiber matrix (SEFM) has been shown to provide durable wound coverage and promote tissue healing as an alternative to traditional biologic allogeneic skin substitutes. Our objective was to evaluate wound application and results for SEFM in head and neck intraoral reconstruction.

Methods: A retrospective case series included patients who underwent surgical resection followed by intraoral SEFM (Restrata®, Acera Surgical, Inc., St. Louis, MO) reconstruction at a tertiary care multidisciplinary head and neck cancer center between January 1, 2022 and January 1, 2024. Reconstructive applications included intraoral full-thickness mucosal defects, exposed musculature, or exposed bone. Serial wound photographs were taken to evaluate wound healing at 1, 4, 8, 12, and 16 weeks after surgical application. Outcomes measured at each time point included wound size, granulation tissue presence, and extent of mucosalization.

Results: In total, 9 patients were included in the study, with a mean age of 67.4. Of them, 7 had tobacco exposure, and 2 patients were previously radiated. Indications for ablation surgery were ameloblastoma, high-grade dysplasia, and squamous cell carcinoma. Follow-up was between 2 to 11 months. The average defect size reconstructed with SEFM was 22.63 ± 7.21 cm2 (5 – 70 cm2). No hematomas or wound complications were encountered. All patients had complete wound healing, with the earliest noted at 3 weeks post-surgery. Scar formation and contracture were acceptable in all cases.

Conclusions: This case series highlights using SEFM for intraoral reconstructive applications with excellent wound healing results. Avoiding patient morbidity associated with split-thickness skin graft is a significant benefit. Further studies with a larger cohort are required.
