Validation of a Low-Cost 3D-Printed Silicone-Based Laryngeal Model for Resident Education

Introduction: In laryngology, trainees are faced with learning an array of surgical techniques that are primarily single-surgeon operations with very low tolerance for surgical error. However, there is little data on using simulation to assist trainees with their laryngeal surgical technique. We sought to validate a realistic low-cost laryngeal simulation model and show the efficacy of this model in residency education to improve surgical technique.

Methods: A low-cost, high-fidelity laryngeal model was created using a 3D-printed mold and multi-layered silicone to mimic vocal fold lesions. Participants were given a series of tasks including a vocal fold lesion resection and creation of a micro-flap for a submucosal lesion excision. Trainees were then provided with an instructional video from a fellowship-trained laryngologist and asked to repeat the same tasks on the model. Performance data was then assessed using validated surveys and blinded expert reviewers.

Results: In total, 18 people completed the simulation. Universally, all individuals expressed unanimous agreement regarding the utility of the simulation experience. All residents agreed that “the simulator helped improve my knowledge” and 93% agreed that “the simulator helped improve my ability to do microsurgical tasks.” In the post-instruction self-evaluation, there was a statistically significant decrease in perceived mental demand (95%CI: 0.37–0.91; p = 0.038), accompanied by a parallel and significant increase in subjective performance (95%CI: 1.51–51.89; p = 0.016) upon repeated attempts. Preliminary objective assessment by a laryngologist similarly showed significant improvements in task-specific scores and overall global operative performance scores on repeated attempt (p = 0.02).

Conclusions: This study demonstrates the usefulness of a low-cost, high-fidelity silicone larynx model in developing laryngeal microsurgical skills. This study is one of the first to demonstrate the benefit of repeated attempts with a laryngeal model leading tangible and quantifiable improvement in residents' subjective and objective skills and proficiency.
