Controlling Nutritional Status Score: A Predictive Marker of Survival in Head and Neck Cancer

Introduction: This study aimed to evaluate the prognostic significance of the preoperative Controlling Nutritional Status (CONUT) score in head and neck squamous cell carcinoma (HNSCC).

Methods: A total of 636 patients with HNSCC who underwent primary radical surgery between 2008 and 2017 were included in this study. The CONUT score was defined as the sum of the serum albumin score, total cholesterol score, and total lymphocyte count score, all measured within 1 week before radical surgery. Cox proportional hazards models were utilized to identify factors associated with survival outcomes. A CONUT-based nomogram was developed for individualized overall survival (OS) prediction.

Results: The optimal cut-off for CONUT to predict 5-year OS and cancer-specific survival (CSS) was determined to be 2. For those with CONUT ≥2, the 5-year OS and CSS were significantly inferior compared to those with CONUT <2 (OS: 58.9% versus 80.4%, p< 0.001; CSS: 74.8% versus 89.6%, p< 0.001). In the multivariate Cox model, a high CONUT score emerged as an independent negative prognosticator of OS (HR: 2.643, 95% CI: 1.813–3.854, p< 0.001) and CSS (HR: 2.886, 95% CI:1.658–5.024, p< 0.001). The CONUT score-based nomogram accurately predicted OS (concordance index: 0.755).

Conclusions: Our study suggests that the preoperative CONUT score is a convenient and effective prognostic marker for HNSCC. Integrating CONUT into the nomogram-based risk stratification model may better categorize patients with poorer oncological outcomes and support decision-making regarding treatment strategies.
