Quantifying Nasal Cartilage Stiffness Through a Novel Measurement Device

Introduction: Nasal obstruction is a widespread issue, with dynamic collapse during inspiration being a primary cause. There is no method to precisely quantify nasal cartilage stiffness. This study employs nasal cartilage calipers offering an accurate measurement of nasal cartilage stress-strain relationships based on force and compression.

Methods: A standard set of digital calipers with a data output port was used, providing 0.01 mm resolution. To measure force, a 100g load cell was fixed to one of the jaws of the calipers. The data from the load cell and calipers were captured by an Arduino UNO and streamed to a computer for further analysis. Custom tips were 3D printed for the jaws of the calipers. Device validation was performed using standardized weights and standardized linear measurements. For each linear distance, a force was applied to the load cell which allowed for the creation of a custom equation representing cartilage stiffness as a function of distance and force. Preliminary clinical testing involved a test nose with multiple trials (n = 3). The measurements were taken using the septum and midpoint between the upper and lower nasal cartilage.

Results: A third order polynomial curve was fitted to the distance and force readings. As the distance reached a minimum against the septum, the force grew. With a stiffer nose, the steeper rise of the curve will slightly flatten out. The test case nose data was plotted and polynomial regression yielded: y = -0.7008x3 + 24.936x2 - 292.84x + 1159.8 and an R² of 0.9943, representing the stiffness of the cartilage as a functional equation.

Conclusions: This novel device provides a quick and functional method of quantifying nasal cartilage strength in patients with dynamic airway collapse. Further testing will involve more trials of healthy noses with an ultimate goal of post surgical rhinoplasties or radiofrequency airway modeling to establish a baseline stiffening after treatment.
