Prognostic Utility of Controlling Nutritional Status (CONUT) Score in Patients With Surgically-Treated Oral Cavity Cancer

Introduction: The present study aimed to assess the prognostic significance of preoperative Controlling Nutritional Status (CONUT) score, which integrates immunocompetence as well as lipid and protein metabolism, in patients with oral cavity squamous cell carcinoma (OCSCC).

Methods: Clinical data from 352 patients who underwent curative surgery for OCSCC at our hospital between 2009 and 2018 were reviewed retrospectively. The optimal CONUT score cutoff was determined through X tile analysis, and the study assessed the significance of associations between the optimal cutoff value and clinicopathological characteristics, as well as survival outcomes.

Results: We identified that a CONUT score of 1 was the optimal cutoff value for predicting OS. The estimated 5-year DFS and OS were significantly shorter in patients with a CONUT score of >1 compared to patients with low-CONUT score of ≤1 (both p < 0.001). Multivariable analysis indicated that the high CONUT score (>1) was an independent negative prognostic factor of OS and DFS (hazard ratio = 1.963 and 2.026, respectively). The prognostic significance of CONUT score remained consistent across subgroups. We developed a CONUT score-based nomogram, which delivered precise OS predictions (concordance index: 0.772).

Conclusions: The preoperative CONUT score may be a valuable prognostic biomarker for patients with surgically treated OCSCC, and the nomogram based on CONUT score may serve as a useful predictive tool in OCSCC management.
