Applying the Platinum Rule to Patient-Centered Care

PROGRAM DESCRIPTION: As opposed to "do unto others as you would have them do unto you," as the golden rule states, the platinum rule asks you to "do unto others, wherever possible, as they would want to be done to them.

Patient center care has become a prevalent model of healthcare delivery and the term is frequently used to connote novel thinking in the patient care interaction. However, the majority of research about patient centered care comes from Western Europe and its offshoots. Moreover, decision-making and open dialogue to exchange information are the two most prominent aspects of patient centered care that we discuss. True patient centered care requires cultural awareness, cultural competence, and cultural humility. Nicole Sheeran’s 2022 study considered culture as an influence on patient preferences.

This workshop would go deeper to look specifically at the US demographics with the worst healthcare outcomes and try to understand the potential barriers to centering the patient as opposed to the ideals generated from misconceptions and bias on the part of providers.

Groups with the poorest outcomes ( in multiple studies) include:
Black
Hispanic (cultural, includes multiracial people)
American Indian/Alaska Native (AIAN)

These are also the groups with lowest representation in Otolaryngology-Head and Neck Surgery. So, part of the workshop will explore the themes of physician-patient race/culture concordance to improve outcomes.

Another group currently experiencing significant isolation and stigmatization are transgender patients seeking care and, by extension, the greater (LGBTQIA) community. This represents another group of voices in the AAO-HNSF that can be amplified to avail ourselves of the reach diversity of experiences and skills present in our specialty in order to frame best practices to deliver more meaningful, equitable, care.

Learning Objectives:

  1. Define person-centered care understanding that we are called to do unto others as they would have us do unto them. Drawing on the work of panelists and others, we will frame how culture influences patient preferences, compliance, and interactions and office advice.
  2. Review disparities in life expectancy and treatment outcomes with the reminder that human beings are 99.9% genetically matched. Review social determinants of health with the understand that bias and discrimination can be powerful determinants.
  3. Offer cultural humility and information to improve our approach to patient centered care. This will be interactive and hopefully we can provide insights through answering audience questions. If time permits, we may run through case scenarios as illustrations.
