Trends in Female Authorship at the American Academy of Otolaryngology–Head and Neck Surgery Foundation Annual Meetings, 2007–2022

Introduction: Practicing female otolaryngologists account for 20% of the national otolaryngology workforce with substantial efforts in recent years to increase gender diversity in academic spaces. Previous studies have established trends in female representation in otolaryngology program committees, executive boards and conference panels, but there is paucity of literature quantifying changes in female authorship in scientific presentations over the last 15 years.

Methods: We present an analysis of female authors at the Annual American Academy of Otolaryngology-Head and Neck Surgery (AAO-HNS) meetings by compiling publicly available scientific oral and poster presentation abstracts from 2007 to 2022. Using ChatGPT-3.5 API, the gender identity of each author was predicted. Secondary variables collected included presentation type (oral or poster presentation), presentation topic and authorship order (first author, presenter, senior author). The trend in female authorship across the years studied was assessed using Cochran-Armitage trend test.

Results: Our analysis included 1,455,644 author occurrences, comprising 25,251 distinct authors and 22,840 abstracts, with 4.3% of gender predictions excluded due to indeterminacy. Female authors were represented in 32.8% of all oral and poster presentations, increasing from 22.2% in 2007 to 34% in 2022 (p < 0.001). While female authors accounted for 37% of presenters and 34.8% of first authors, their representation as senior authors was 25.6%. Gender concordant author-dyads were more frequently observed in earlier years where 74.3% of first and senior authors were identified as same gender (M-M or F-F) in 2007. Gender concordance decreased to 60.99% of author dyads in 2022.

Conclusions: Representation of female authors at annual AAO-HNS meetings has increased from 2007 to 2022. These promising trends likely reflect the changing demographics of otolaryngology trainees and their mentors. Future studies are charged with evaluating the most efficacious methods of promoting gender diversity, to continue increasing female representation in otolaryngology research.
