Evaluating the Joint Committee on Infant Hearing (JCIH) Newborn Hearing Screening Protocols Pre and During COVID-19 Within Washington, DC

Introduction: Results of the Newborn Hearing Screening (NHS) program in Washington, DC were reviewed and analyzed from the DC Oz e-screener Plus Database pre-COVID and during COVID to determine if they met the feasibility attainment of 70% as declared by Joint Committee on Infant Hearing (JCIH). The study’s aim is to determine the possible factors affecting the Lost to Follow-up (LTF) rate and potential ways to improve the LTF rate, specifically in the areas of Washington, DC that are the most affected.

Methods: Analysis of NHS reports for factors impacting LTF were analyzed from March 2019 to March 2021. Data collected included demographics (age, sex, birth hospital, median income, zip code) and DC ward of residence. Multivariate and univariate regression analysis were used to identify factors pre (March 1, 2019-March 1, 2020) and during COVID (March 2, 2020-March 1, 2021) periods.

Results: Out of 15,661 infants, 830 did not pass their initial hearing screening. Of the 830, 406 were in the pre-COVID group, and 424 infants were in the during COVID group. Of the 830 infants, 388 (46.7%) infants did not pass their Outpatient (OP) screening. Of the 388 infants who did not pass their OP screening, 342 (88.1%) of them did not receive a Diagnostic Auditory Brainstem Response (Dx ABR). Of the 342, 205 (90.7%) in the pre-COVID and 137 (83.5%) in the during-COVID group. Birthing hospital was found to be a significant factor for those who did not pass OP NHS and did not receive a Dx ABR (2.6% and 6.0%, respectively). Of the total cohort, gender, birthing hospital, income and testing period were significant on both multi and univariate analysis for failed screenings and LTF (2.3%, < 0.1%, 3.5%, 0.3%). Ward of residence was also significant on univariate analysis (p < 0.001).

Conclusions: There are many factors impacting LTF rates and future follow-up when using the NHS protocol recommended by Early Hearing Detection and Intervention (EHDI). This study will act as the basis for implementing planned improvement strategies for reducing the LTF rate within Washington, DC.
