Statewide Social Vulnerability Index (SVI) as a Predictive Factor in Oropharyngeal Carcinoma

Introduction: Social Vulnerability Index (SVI) is a novel composite measure developed by the CDC composed of key social determinants of health. This study investigates the association of SVI with outcomes of patients with oropharyngeal squamous cell carcinoma (OPSCC) to elucidate the utility of SVI in head and neck cancer care.

Methods: This study included individuals from Illinois diagnosed with OPSCC at our institution from 2007 to 2022. The state-level SVI measured neighborhood-level disadvantage, utilizing patients' residence at the time of diagnosis. Patient neighborhoods were mapped across a choropleth map representing social vulnerability levels across US Census tracts. Statistical analyses involved Fisher's exact test for categorical variables. The log-rank test was used to evaluate overall survival and recurrence-free survival.

Results: The higher SVI (more disadvantaged) group consisted of a significantly greater proportion of Black race (p < 0.01) and Hispanic/Latino ethnicity (p = 0.05). On multivariable analysis, non-White race was significantly associated with increased odds of advanced T-stage at the time of diagnosis (OR = 5.52, p < 0.001). Non-Medicare and non-private insurance status was associated with a significantly increased hazard of mortality (HR = 10.3, p = 0.005) and disease recurrence (HR = 6.8, p = 0.005) compared to the Medicare and privately insured groups.

Conclusions: State-level social vulnerability was not found to be associated with mortality or recurrence but may be helpful in stratifying clinicodemographics of Illinois OPSCC patients. This study provides insight into disparities affecting our institution’s patient population and allows us to better discern which disadvantaged neighborhoods may need preventative interventions.
