PROGRAM DESCRIPTION: Primary hyperparathyroidism caused by a single adenoma is often a simple minimally invasive resection. However, bilateral exploration of the neck can be expected or unexpected while performing parathyroidectomies. Each of the causes of hyperparathyroidism, from the development of single-adenoma or multi-gland disease have similar processes but distinct preoperative work-up, medical and surgical management, and immediate post-operative surveillance. This expert panel will review the important aspects, nuances, and unique challenges of treating this multifaceted disease process, with particular focus on when and how to do a bilateral exploration with today’s available tools. This is a case-based presentation filled with evidence-based practices. The audience can expect new cases if they have attended this panel in the past.

Learning Objectives:

Differentiate between hereditary primary, secondary and tertiary hyperparathyroidism: When to expect and plan for bilateral neck exploration for parathyroid surgery, Non-localizing parathyroid adenoma, Hereditary primary hyperplasia vs non-hereditary primary hyperplasia.
Understand the work up for multi-gland disease in hyperparathyroidism, with attention to secondary and tertiary hyperparathyroidism and post-operative medical management (include hungry bone syndrome).
What happens if preoperative localization imaging is a false positive during surgery and the surgical pearls and strategy when performing bilateral neck exploration?
