The Impact of Cochlear Implant Use on Auditory Outcomes Over Time

Introduction: Increased daily cochlear implant (CI) usage in pediatric patients has been shown to improve early auditory outcomes. With audiological testing, children advance to more difficult tasks after reaching the ceiling for a specific test. Some measures used in order of increasing difficulty include Northwestern University-Children’s Perception of Speech (NU-CHIPS), Word Intelligibility by Picture Identification (WIPI), Cochlear-Nucleus-Consonant (CNC), and BabyBio. This study aimed to evaluate the effect of hearing hour percentage (HHP), a measure of daily CI usage, on these specific auditory outcomes over time.

Methods: This was a retrospective review from 2013-2023 of pediatric CI recipients at two institutions with at least 6 months of CI use. Patients were excluded if they had no available audiologic scores, were nonverbal, or had comorbidities impacting cognition. Demographic characteristics and audiologic history were collected. HHP was calculated using patients’ datalogging hours. Audiologic measures included NU-CHIPS, WIPI, CNC and BabyBio tests. Changes in audiologic scores over time were correlated by Spearman’s Rank correlation.

Results: A total of 111 patients were reviewed and 68 patients met inclusion criteria. Average age at CI implantation was 3.7 ± 4.1years. Most recent follow up was between 0.5 to > 5 years. At the most recent time point, NU-CHIPS (n = 34, r = 0.22, p = 0.026), WIPI (n = 29, r = 0.41, p = 0.023), and CNC (n = 62, r = 0.43, p < 0.001) correlated with HHP, while BabyBio (n = 57, r = 0.24, p = 0.12) did not. No tests significantly correlated with change over time: NUCHIPS (n = 14, r = -0.22, p = 0.62), WIPI (n = 14, r = -0.49, p = 0.12), CNC (n = 37, r = 0.13, p = 0.51), and BabyBio (n = 35, r = 0.26, p = 0.22).

Conclusions: Higher average HHP was significantly associated with improved CNC, WIPI, and NU-CHIPS scores at the most recent time point. These findings suggest both word and speech recognition performance may correlate with higher HHP. Higher complexity tasks requiring increased auditory functioning (BabyBio) may require longer device use before demonstrating similar correlation of increased scores with higher HHP.
