Ibuprofen and Preoperative Pain Counseling Reduce Opioid Use Following Routine Endocrine Surgery

Introduction: Nearly 10% of opioid-naïve patients continue to use opioids 1 year after receiving a prescription for low-pain short-stay surgery. On average, 21 opioid tablets are reportedly prescribed following endocrine surgery, despite literature showing that many endocrine surgery patients require no opioid pain medications.

Methods: In September 2022, our single institution standardized the perioperative pain management of endocrine surgery patients with routine ibuprofen prescribing and detailed counseling about pain. We followed the next 80 patients as a prospective cohort with postoperative surveys about pain and medication use. These patients were compared to 160 patients who had routine endocrine surgery immediately prior to the official standardization using univariate and multivariate analysis.

Results: A standardized pain management routine reduced opioid prescriptions by 46.6% for routine endocrine surgery patients (20.2% vs 11.0%, p < 0.01). On multivariate logistic regression, receiving education about pain at discharge (OR 0.37, p < 0.05) and older age (OR 0.97, p < 0.01) were associated with fewer opioid prescriptions while anxiety (OR 2.77, p < 0.05) and drain placement (4.61, p < 0.001) were associated with more opioid prescriptions. Out of of surveyed patients, 66.6% completed at least one questionnaire and most patients complained of headache, fatigue, soreness at IV sites, or a sore throat as opposed to pain from the surgical site.

Conclusions: Standardized counseling about pain, including expectations and adjunctive measures, reduces opioid prescriptions in patients undergoing routine endocrine surgery. Further study is needed to better understand which patients require opioid prescriptions and how perioperative pain counseling alters patient experiences with postoperative pain.
