Prognostic Significance of Systemic Inflammatory Index in Head and Neck Cancer

Introduction: The prognostic relevance of the Systemic Inflammatory Index (SII) in head and neck cancer is increasingly recognized. This study aims to evaluate the prognostic value of SII and to establish its correlation with overall survival in patients with head and neck cancer.

Methods: We conducted a retrospective analysis of head and neck cancer (HNC) patients treated at a single tertiary care site. SII was calculated for each patient within 1 month prior to surgery. SII was calculated as (neutrophil × platelet ) ÷ lymphocyte. Receiver Operating Characteristic (ROC) curves were utilized to determine the optimal cutoff value of SII for prognostic significance. Kaplan-Meier survival curves were plotted to compare survival based on the SII cutoff, and a Cox proportional hazards multivariate model was employed to adjust for potential confounders.

Results: A total of 77 patients were included in the study, 60 males and 17 females. The average age of the patients was 65.8 years. The ROC analysis identified SII cutoff of high SII over 2,098, stratifying patients into groups with significantly different survival outcomes (AUC = 0.6). Kaplan-Meier analysis indicated a notable survival difference between these groups with high SII demonstrating median survival of 3.1 years compared to low SII with median survival of below 1.5 years (p < 0.05). In survival analysis, a Cox Proportional Hazards model identified SII as a significant predictor of survival (HR = 6.47, 95% CI 1.04 – 41.68; p< 0.05) when adjusting for age, gender, BMI, tobacco use, and alcohol use. In the multivariate analysis, a high SII was found to be an independent prognostic factor for overall survival (OR= 1.75, 95% CI 1.25-2.46; p < 0.05).

Conclusions: The findings of this study underscore the prognostic significance of the Systemic Inflammatory Index in head and neck cancer. The association between SII and overall survival highlights the potential of SII as a marker for further research and its possible future role in the management of HNC.
