Clinical Dharacteristics of Central Compartment Atopic Disease Compared With Other Subtypes of Chronic Shinosinusitis

Introduction: Central compartment atopic disease (CCAD) is an emerging phenotype of chronic rhinosinusitis with nasal polyposis (CRSwNP). CCAD may present as polypoid changes of the entire central sinonasal compartment while lateral sinus remains relatively normal. This study compares the clinical characteristics of CCAD relative to CRSwNP and chronic rhinosinusitis without nasal polyposis (CRSsNP).

Methods: Retrospective analysis was performed at a tertiary rhinologic center for patients who were treated with bilateral endoscopic sinus surgery for CRS with or without nasal polyp from January 2022 to December 2023. Patients were allocated into 3 groups: CCAD, CRSwNP, and CRSsNP. The demographic data, presence of allergy or asthma, Serum total immunoglobulin E, and serum eosinophil, JESREC score, SNOT-22, olfactory function, taster or non-taster status of the included patients were compared among the three groups.

Results: A total of 270 patients were analyzed (CRSsNP, n = 56; CRSwNP, n = 140; CCAD, n = 74). Prevalence of allergy was significantly higher in CCAD (45.9%), CRSwNP (41.4%), when compared with CRSsNP (25%). Asthma prevalence was highest in CCAD (16.2%) compared with CRSwNP (14.3%) and CRSsNP (3.6%). Serum total immunoglobulin E, and serum eosinophil, JESREC score were significantly higher in CCAD than in CRSwNP or CRSsNP. Patients in CCAD group showed more olfactory disturbance than other groups. The percentage of non-taster was much lower in CCAD group (11.1%) than others (20.6%).

Conclusions: CCAD has distinct clinical characteristics compared with CRSwNP and CRSsNP. It represented a high prevalence of allergy and asthma and also showed a strong association with the markers of type 2 inflammation and olfactory disturbance.
