Treatment Outcomes for Type 1 Thyroplasty in Geriatric Patients: A Descriptive Study

Introduction: Goretex® thyroplasty is utilized often to manage various laryngeal disorders presenting with glottic insufficiency. There is more to understand of its role in the management of older patients. We aimed to describe our geriatric patient population who underwent thyroplasty for glottic insufficiency.

Methods: After IRB approval, retrospective chart review of all patients undergoing thyroplasty between 2013-2023 was performed. Patient demographics, procedural details, anesthesia records and postoperative complications were reviewed.

Results: A total of 49 patients were noted to be 65 years and older; 26 male and 23 female, with an average age of 73 years old (range: 65-83). Of them, 30 patients underwent unilateral and 19 bilateral surgery. Precedex and propofol were the most commonly used anesthesia medications. No intraoperative events required surgeon to abort procedure. Vocal fold paralysis was diagnosed in 63.3%, presbylarynx in 34.7%, and glottic insufficiency due to other reasons such as Parkinson’s disease, MS and intubation, in 26.5%. 58% of patients with vocal fold paralysis had iatrogenic injury; stroke and lung cancer being other causes. Most (83%) of the cohort received other interventions prior to thyroplasty, with 69.5% undergoing voice therapy. Others underwent vocal fold injections, with carboxymethylcellulose being the most common filler. Median time between injection laryngoplasty and thyroplasty was 10.5 months. A sum fo 34 patients reported satisfaction and continued symptom improvement after thyroplasty. Another 5 patients experienced initial symptom improvement; however, new-onset symptoms began within one year after surgery. Additionally, 4 patients who were not satisfied with initial results underwent revision or opposite unilateral thyroplasty and reported symptom improvement. One patient experienced postoperative infection that warranted implant removal. Postoperative voice therapy was completed by 17 patients.

Conclusions: Thyroplasty in the geriatric patient population is effective in eliminating symptoms of glottic insufficiency due to various etiology.
