The Clinical Efficacy and Safety of Highly Selective Vidian-Branch Neurectomy in Intractable Allergic Rhinitis

Introduction: Intractable allergic rhinitis (iAR) refers to a condition where symptoms remain uncontrolled despite undergoing standard drug therapy. Endoscopic vidian neurectomy has provided new hope for iAR patients, but postoperative complications are common. We have modified this procedure, selectively cutting the fibers behind the vidian ganglion to reduce postoperative complications, and named it " highly selective vidian branches neurectomy." This study analyzes the efficacy and safety of highly selective vidian nerve branches neurectomy in treating iAR.

Methods: A total of 205 iAR patients were recruited and divided into 3 groups: highly selective vidian branches neurectomy (group A), endoscopic vidian neurectomy (group B), and conservative treatment (group C). The rhinoconjunctivitis quality of life questionnaire (RQLQ), total nasal symptom score (TNSS) and medication score (MS) were compared before and after treatment. All patients were follow-up for more than two years, and the efficacy and postoperative complications were compared.

Results: After a two-year follow-up, a total of 195 patients were included, with 59 patients in group A, 71 in group B, and 64 in group C. Post-treatment assessments revealed significant reductions in the RQLQ, TNSS, and MS levels in both groups A and B compared to baseline levels. However, no significant changes were observed in group C. Furthermore, symptoms were alleviated in all patients in groups A and B after surgery. The improvements in RQLQ, TNSS, and MS were higher in groups A and B compared to group C. In group A, only 2 patients reported nasal dryness during the first month post operation. However, in group B, 9 patients experienced dry eyes and 5 patients suffered postoperative bleeding, and 6 patients suffered dry eyes for more than 1 year.

Conclusions: Both endoscopic vidian neurectomy and highly selective vidian branches neurectomy were effective in controlling symptoms of iAR. However, highly selective vidian branches neurectomy appears to be a safer option, associated with fewer postoperative complications.
