Automatic Detection of Torsional Nystagmus Using the CAVA System

Introduction: The CAVA device (https://www.uea.ac.uk/groups-and-centres/cava-project) is a medical diagnostic device that has been developed to allow the automatic diagnosis of vertigo in symptomatic individuals by using electro-oculography to continuously record their eye movements over a 30-day period. Benign Paroxysmal Positional Vertigo is a common vestibular condition, so it is essential that any automated process to diagnose the cause of vertigo is able to identify short periods of torsional nystagmus.

Methods: In total, 15 individuals experiencing vertigo as a consequence of Meniere’s disease, vestibular migraine, and Benign Paroxysmal Positional Vertigo (BPPV) were monitored nearly continuously (23 hours in 24) using the CAVA device for periods of up to 30 days. Vertical and horizontal eye movement waveforms were downloaded from the CAVA device and analyzed automatically for nystagmus in one or both components. These traces were in turn analyzed for showing torsional nystagmus, using a novel detection algorithm that was validated against clinical judgment.

Results: A total of 6,920 20-second duration episodes of nystagmus were recorded. Of these, 184 were deemed to contain torsional nystagmus, with clockwise movement accounting for 87% of the total and anti-clockwise 13%. Of these episodes, 160 came from patients with Ménière's Disease and the 6 BPPV patients contributed only 14 episodes.

Conclusions: The novel nature of the CAVA device means that it is well-suited to the task of reliable automatic detection of torsional nystagmus in the patient's day-to-day life. Further collection of data from subjects using CAVA will help to understand nystagmus forms better and to diagnose inner ear conditions.
