Diagnostic Accuracy of Nasal Endoscopy and Contrast Magnetic Resonance Imaging in COVID-19-Associated Mucormycosis

Introduction: The cross-sectional study was done to evaluate the accuracy and correlation of diagnostic nasal endoscopy (DNE) and contrast-enhanced magnetic resonance imaging (CEMRI) in comparison to potassium hydroxide calcofluor white mount (KOH) in CAM.

Methods: In total, 102 consecutive patients with clinical suspicion of CAM were enrolled in the study during a 3 month period and were subjected to DNE and biopsy for KOH mount. CEMRI was performed in 60 patients. Results were analyzed considering KOH positive as a case of CAM. Out of 102 study patients, 49 (48%) had positive DNE findings, out of which 47 (96%) came positive for KOH mount while 2 were negative. Normal DNE was shown in 53 (52%), out of which 17 (32%) came positive for KOH mount while 36 were negative. MRI was done in only 60 subjects, out of which positive findings were observed in 51 (85%) patients.

Results: The sensitivity and specificity of DNE as compared to KOH mount were 73.4% and 94.7% respectively. The positive predictive value, negative predictive value, and accuracy were 95.5%, 67.9%, and 81.37% respectively. The sensitivity and specificity of CEMRI as compared to KOH mount were 87.5% and 50% respectively. The positive predictive value, negative predictive value, and accuracy were 96.07%, 22.22%, and 85% respectively.

Conclusions: CEMRI misses fewer cases than DNE. Nasal endoscopy, biopsy, and imaging can reliably detect the majority of cases of CAM. In cases of normal endoscopy, imaging and biopsy should be advised to reduce false-negative results in all those with high clinical suspicion of CAM.
