Three-year Outcomes Following Temperature-Controlled Radiofrequency Posterior Nasal Nerve Ablation Treatment for Chronic Rhinitis

Introduction: Chronic rhinitis is characterized by inflammation of the nasal cavity lining, resulting in symptoms such as rhinorrhea, sneezing, nasal congestion, cough and postnasal drip, Temperature-controlled frequency (TCRF) ablation of the posterior nasal nerve (PNN) is a minimally invasive procedure shown to be safe and effective in improving symptoms of chronic rhinitis through 2 years post-procedure.

Methods: This is a prospective, single-arm, open-label, multicenter study evaluating the long-term efficacy, safety, and durability of TCRF PNN ablation in patients with chronic rhinitis. The study included patients aged 18 to 85 years across 16 centers in the United States and 3 centers in Germany who were followed through 3-year post procedure. Outcome measures included rTNSS, postnasal drip and cough scores, and MiniRQLQ. Adverse events were reported through 3 years post procedure.

Results: Of the 138 patients enrolled, 129 received TCRF treatment, and 101 completed follow-up through 3 years. Compared to baseline, rTNSS significantly reduced from a mean score of 7.6 (95% CI, 7.4 to 7.9) to 3.2 (95% CI, 2.7 to 3.7) at 3 years, representing a 67.3% decrease, a mean change of -4.6 (95% CI, -5.2 to -4, p < 0.001). The majority (82%) of the patients were considered responders to treatment at 3 years (defined as having an MCID of ≥30% decrease post procedure). At the 3-year follow-up, cough symptoms were reduced from a mean score of 1.3 at baseline to 0.4 at the 3-year follow-up (mean change, -0.9; p < 0.001) and postnasal drip symptoms were reduced from 2.4 at baseline to 1.2 (mean change, -1.2; p < 0.001). Mean MiniRQLQ quality of life scores significantly decreased from a mean score of 3 at baseline to 1.2 at 3 years, representing a 60% decrease (improvement). No serious device or procedure-related adverse events were reported during the study.

Conclusions: A single TCRF PNN treatment resulted in durable long-term improvement in chronic rhinitis symptoms and patient quality of life through 3 years.
