Olfactory Outcomes Following Endoscopic Sinus Surgery for Chronic Rhinosinusitis With Nasal Polyposis

Introduction: The purpose of this study is to evaluate subjective and objective olfactory outcomes in patients undergoing endoscopic sinus surgery (ESS) for chronic rhinosinusitis with nasal polyps (CRSwNP) and to identify predictors of olfactory improvement following ESS.

Methods: This prospective study included adult patients undergoing ESS for CRSwNP at a single tertiary care center from July 2022 to May 2024. Olfactory status was assessed using the Sinonasal Outcome Test (SNOT-22) and the Sniffin’ Sticks odor identification test preoperatively and at 1, 3, and 6 months postoperatively.

Results: In total, 46 patients (41% female, mean age: 47.8 ± 13.8) were included. Among them, 47% had asthma, 47% had a history of previous sinus surgery, and 26% had aspirin-exacerbated respiratory disease. The average baseline preoperative Sniffin’ Sticks score was 5.7 odors identified (out of 16). The average preoperative SNOT-22 smell score of 4.2 ± 1.2. At 1, 3, and 6 months postoperatively, there were significant decreases in SNOT-22 smell scores of 2.2, 2.4, and 2.3 points, respectively (p < 0.001). The Sniffin’ Sticks identification scores improved by 2.5, 2.7, and 1.1 points at 1, 3, and 6 months, respectively, with statistically significant improvements at 1 and 3 months (p < 0.001), but not at 6 months (p > 0.05).

Conclusions: This study uniquely evaluates olfaction in patients with CRSwNP at regular postoperative intervals. While significant improvement in subjective olfaction was sustained up to 6 months postoperatively, objective improvement was only seen at 1 and 3 months postoperatively.
