Investigating Potential Synergies Between Hypoglossal Nerve Stimulation and Nasal Surgery in Obstructive Sleep Apnea

Introduction: Hypoglossal nerve stimulation (HGNS) surgery has shown improvement in obstructive sleep apnea outcomes, while nasal surgery has shown questionable potential. The integration of these procedures may enhance effectiveness and patient outcomes. The goal of this project is to determine if HGNS combined with nasal surgery provides synergistic benefits to sleep outcomes.

Methods: A single-center retrospective review was conducted on patients who underwent HGNS alone or HGNS in combination with nasal surgery from 2022-2024 at a tertiary medical center. Preliminary statistical analysis was performed using independent and paired sample t-tests to measure patient-reported survey outcomes and polysomnogram data points. The database is actively growing, and updated data will be included in the final project.

Results: A total of 21 patients were included in the nasal with HGNS (N-HGNS) group, while 17 were included in the HGNS group (HGNS). At 2-month follow up, Apnea-Hypopnea Index (AHI) (40 vs 21, p = 0.003), Epworth Sleepiness Scale (ESS) (9.9 vs 6.7, p < 0.001) and Nasal Obstruction Symptom Evaluation (NOSE) (38 vs 21, p < 0.001) scores were improved. The percent change in AHI was -55% in the combined surgery group compared to -7% in the HGNS-only group (p = 0.085). NOSE scores were improved even in patients that did not undergo nasal surgery (41 vs 27 pre and postop, p = 0.002).

Conclusions: This preliminary data indicates that HGNS combined with nasal surgery has a greater impact on improving postoperative AHI, with results approaching statistical significance. Interestingly, HGNS alone subjectively improves nasal breathing, showing a relationship that has not yet been described in HGNS outcome data. Further analysis will focus on breakdown of specific nasal procedures and detailed polysomnogram data to better elucidate these findings.
