Gender Affirming Surgery of the Head & Neck for the Comprehensive Otolaryngologist

PROGRAM DESCRIPTION: The lecture will be divided into four parts, each concentrating on the impact that gender affirmation surgery of the head and neck has on subsequent assessment and surgical intervention of unrelated (or related) otolaryngological complaints encountered by a comprehensive otolaryngologist. We will also be considering non-surgical issues unique to this population. Topics include the impact of gender affirming feminization and masculinization laryngoplasty on the airway including implications for endotracheal intubation; LMA mask anesthesia; dysphonia; laryngeal anatomy and emergency airway concerns. Additionally, the consequences of facial feminization and masculinization on subsequent cosmetic and non-cosmetic head & neck procedures, as well as potential delayed post-operative complications related to gender affirming procedures performed elsewhere will be addressed. Thirdly, special pre-, peri-, and post-operative considerations for transgendered and non-binary patients on gender affirming hormone therapy will be discussed as it relates to medical and surgical otolaryngology. Finally, there will be a brief presentation of issues regarding gender dysphoria, unrecognized concurrent psychiatric disorders and the patchwork of anti-LGBTQ+ state laws in regard to medical decision making and informed consent. These topics are relevant to the comprehensive otolaryngologist and allied health professionals who may have less experience navigating the unique circumstances inherent to this underserved population.
