(0212) Developing Standardized Nomenclature for Tracheostomy and Laryngectomy Supplies: Streaming Workflow, Reducing Waste, and Preventing Harm

Introduction: Tracheostomy nomenclature is variable across each manufacturer and distributer and furthermore, neither naming system is aligned with clinical language used in daily practice. Additionally, our electronic systems for ordering supplies into supply chain (People Soft) and ordering supplies to the patient's bedside (Epic) also do not have unified nomenclature and do not interface. At NYULH, this has led to incorrect supplies at bedside, sentinel events (insertion of wrong device in patient) patient harm, and wasted time and resources.

Methods: A small cross-functional workgroup was identified in February 2023, including otolaryngology, speech language pathology, clinical value analysis (nursing), epic, and and supply chain management (SCM). SCM data team pulled all items in the system with "trach" in the item description resulting in 490 items inclusive of laryngectomy supplies and HME. A nomenclature system was agreed upon and vetted and applied to each line item. Content was shared with Epic Orders Team and SCM Data Team and 217 new line items were created.

Results: Shortly after implementation, the new nomenclature and ordering workflow received positive feedback from a variety of disciplines, including nursing, supply chain leadership, and ENT providers. Since April 2023 when the project was implemented, there have been no related PSI events to improper supplies delivered to patient rooms/placed in patients. It has been noted there is less confusion regarding patient supplies and necessary orders needed since implementation of the new nomenclature system.

Conclusions: Standardized nomenclature that is clear and consistent ensures effective communication and limits confusion when referencing products across disciplines. There has been a significant improvement without our hospital system relating to tracheostomy and laryngectomy supply ordering workflows, and in regards to PSI events associated with patient harm. We developed, and successfully implemented a new nomenclature system across our enterprise targeting a high risk, frequently used product.
