(0906) Benign Paroxysmal Positional Vertigo: Clinical Characterstics and Serum Vitamin D Levels at a Tertiary Hospital

Introduction: BPPV is the most frequent peripheral vestibular end organ disorder with a cumulative incidence in the general population of 10% and lifetime prevalence of 2.4%. The study aims to characterize the clinical and serologic features related to our BPPV patients at tertiary hospital.

Methods: A descriptive and transversal study was conducted during January 2022 to January 2024; it included patients with vertiginous symptoms compatible with BPPV and positive diagnostic test maneuvers (Dix Hallpike and supine roll test). Patients with positive vertiginous symptoms triggered by diagnostic test maneuvers with or without nystagmus were treated with repositioning maneuvers (Epley, Lempert, and Semont) according to the semicircular canal stimulated. Patients follow up was 7 days after first repositioning maneuver with serum 25-hydroxyvitamin D levels.

Results: The most prevalent comorbidity was arterial hypertension in 37.6%.
Recurrence risk association was estimated for the antecedent of hypertension showing a lower rate of recurrence. (OR .09 [95%CI, .01-.7]) The presence of T2DM (OR 2.79 [95%CI, 1.08-7.17] and multi-canal involvement (OR 3.7 [95%CI, 1.1- 12] increased the risk of need more than one reposition treatment in a single canal. The frequency of secondary BPPV was 32.5%. In the secondary BPPV group, the most prevalent cause was antecedent of head trauma 7.7%, followed by sudden neurosensorial hearing loss, Ménière's disease, osteoporosis, vestibular neuritis, previous ear surgery, prolonged bedrest, and dental procedures with a frequency of 6%, 5.1%, 5.1%, 3.4%, 3.4%, 0.8%, and 0.8% respectively. Multi-canal involvement in VPPB was present in 12.2%.
The most common 25-hydroxyvitamin D level was insufficiency (42.9%), followed by deficiency, normal, and severe deficiency with a frequency of 32.9%, 20%, and 4.3% respectively. Pretreatment DHI scale score decreased posterior reposition technique 62 (38-78.5) vs 20 (4-48); (p < 0.001).

Conclusions: Knowing the comorbidities and clinical characteristics of our patients with BPPV we can improve and predict the number of reposition needed to have a successful treatment.
