Vestibular Schwannomas in the Pediatric Patient: A Population Database Study

Introduction: Previous research on neurofibromatosis type 2 (NF2) vestibular schwannoma (VS) has been described in children, but little is known about how sporadic VS present in children as compared to adults. This study aims to characterize differences between children and adults with VS and differences in sporadic versus NF2 tumors in the pediatric population.

Methods: Data from the TriNetX Research Network (2004-present) was queried for pediatric (age ≤ 21; VS-peds) and adult (age > 21; VS-adults) patients with VS. The VS-peds cohort was further subdivided into NF2 (NF2-peds) or sporadic mutation (sporadic-peds). Primary outcomes were hearing loss, and pre and post-VS treatment tinnitus, dizziness, and headache (6 months – 3 years). Secondary outcomes included pursuit of surgery and postoperative complications. Outcomes were compared between the pediatric and adult cohorts, as well as the NF2-peds and sporadic-peds cohorts.

Results: The VS-peds cohort (n = 231) had a lower prevalence of pre-treatment tinnitus (29% vs 42%, p < 0.0001) and dizziness (18% vs 38%, p < 0.0001) compared to VS-adults (n = 25,413). History of pre-treatment headaches was more prevalent in VS-peds compared to adults (35% vs 22%, p = 0.0019). There was no difference between cohorts regarding the prevalence of post-treatment tinnitus and headache, post-diagnosis HL or those who underwent surgery. There was a higher prevalence of dizziness following VS diagnosis in adults compared to VS-peds (13% vs 6%, p = 0.0007). When comparing sporadic-peds (n = 122) to NF2-peds (n = 105), sporadic-peds had a higher proportion of females (54% vs 35%, p = 0.0393), while NF2-peds had a higher proportion of Hispanic patients (28% vs < 8%, p = 0.002).

Conclusions: VS in children can present differently compared to adults in addition to what type of VS. Like adults, a significant portion of children continue to have symptoms for months to years following their diagnosis. Although pediatric sporadic and NF2 tumors present similarly, demographics differed between cohorts.
