Changes in Wearable Sleep Technology Metrics During Hypoglossal Nerve Stimulation Uptitration

Introduction: Longitudinal changes in sleep metrics during hypoglossal nerve stimulation (HNS) therapy uptitration is unknown and has potential for monitoring obstructive sleep apnea (OSA) treatment response. This study pilots use of a wearable consumer sleep technology (CST) to assess changes in sleep metrics in response to HNS uptitration.

Methods: In this case series, HNS patients began wearing a CST (Oura Ring) at implant activation. CST outcome metrics included total sleep time (TST), wake time after sleep onset (WASO), average heart rate (AvHR), and heart rate variability (HRV). We used linear mixed effect and multivariate regression models controlling for age, sex, and body mass index (BMI).

Results: A total of 14 participants completed the study with 1,662 nights of data; 6 patients had ≥150 days of CST data. A multivariate analysis showed a significant decrease in TST (r -0.106), WASO (r -0.212), and HRV (r -0.091) with increasing HNS amplitude over time (p < 0.001). TST and HRV positively correlated with age (0.166 and 0.239, p< 0.001). WASO negatively correlated with age (-0.224, p< 0.001). On linear mixed model analysis, HNS amplitude was negatively associated with WASO (r -0.683, t = 3.715, p< 0.005), AvHR (r -0.104, t = 2.238, p< 0.005) and HRV (r -0.684, t = 2.238, p < 0.005). On analysis of each individual subject, changes in HNS amplitude were significantly correlated with cardiac parameters AvHR in 43% (6/14) and HRV in 21% (3/14) of participants demonstrating potential individual susceptibilities to cardiovascular changes associated with OSA.

Conclusions: In this case series, HNS amplitude correlated with TST, WASO, AvHR, and HRV as tracked by a wearable CST. Future studies are needed to understand the effects of HNS therapy on sleep metrics and to define the potential utility of CSTs in longitudinal OSA care.
