Sensorineural Hearing Loss and Phosphodiesterase-5 Inhibitors: A Systematic Review

Introduction: Sensorineural hearing loss (SNHL) is a condition that can profoundly affect an individual's quality of life. Recent studies have suggested that a possible connection exists between the utilization of phosphodiesterase-5 inhibitors (PDE-5i) and the emergence of SNHL. This systematic review seeks to consolidate the existing evidence, providing clarity on the relationship between PDE-5i use and the risk of developing SNHL.

Methods: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) Protocol, PubMed, Embase, and Web of Science were queried by two independent reviewers for articles published from inception to December 2023 describing hearing loss in individuals taking PDE-5is.The methodological index for non-randomized studies (MINORS) criteria was used to assess quality of studies.

Results: Out of the initially identified 190 abstracts, 8 met the inclusion criteria, comprising a total of 58 patients. The reported age of patients ranged from 37 to 62 years. Among the included studies, two involved female participants (n = 9), while the remaining studies exclusively focused on male patients (n = 49). The most commonly used PDE-5 inhibitors were sildenafil, vardenafil, and tadalafil. All patients experienced sensorineural hearing loss, ranging from moderate to profound, with 5 patients (9%) noting tinnitus and vertigo in addition to the hearing loss. One patient experienced bilateral hearing loss 15 days after Sildenafil consumption. Corticosteroid therapy was the exclusive treatment in all cases. Long term outcomes following cessation of the medication were available in 26 cases, 3 patients (11%) had no improvement, 21 patients (81%) fully recovered, and 2 patients (8%) had incomplete resolution in hearing loss.

Conclusions: This review highlights the need for monitoring auditory function in patients prescribed PDE-5i and it is recommended to inform patients about this potential risk. To establish a definitive causal link and comprehend the underlying mechanisms, further research with larger samples and control groups is deemed necessary.
