Grafts, Flaps, and Stuff for Tympanoplasty and Mastoid Obliteration

PROGRAM DESCRIPTION: The aim of chronic ear surgery is to obtain a safe, functional, dry and self-cleaning ear. In addition to disease removal and mastoid bone drilling, skillful handling of the soft tissue is essential to assure optimal results. Several reconstructive techniques that include biological materials such as autografts and flaps, as well as xenografts and non-biological materials have been tried and tested to achieve this goal with varying degrees of success. This course will focus on the handling of the soft tissue in otologic surgery. Two experienced otologic surgeons will review their (1) indications and technique of tympanoplasty and endaural flaps; (2) indications and harvesting techniques of fascia, perichondrium and cartilage autografts; (3) surgical anatomy, indications and technique of regional flaps for mastoidectomy reconstruction; and (4) indications of commercial xenografts. Particular attention will be given to the design of the skin incision based on the anticipated graft or flap. The authors will present and comment on real cases illustrated by photographs and surgical videos.

Learning Objectives:

  1. Learn to select among different types of incisions and tympanomeatal flaps based on factors such as the characteristics of the tympanic membrane perforation and the type of middle ear disease.
  2. Learn the proper harvesting techniques of common grafts such as the innominate temporalis fascia, deep temporalis muscle fascia, and conchal and tragal cartilage and perichondrium.
  3. Learn the surgical anatomy and harvesting techniques of composite flaps (i.e., combination of fascia, periosteum, and muscle) used for mastoid reconstruction and mastoid obliteration.
