Building a Quaternary Head and Neck Oncology Practice in a Community Hospital

PROGRAM DESCRIPTION: Advanced extirpative and reconstructive head and neck surgery, with microvascular reconstructive expertise has historically been performed in mid-large city, often metropolitan academic centers. Utilization of the trainees, including medical students, residents, and fellows lends vital manpower and aid to a complex and sensitive patient population and surgical subspecialty. In addition, the academic environment traditionally attracts more comprehensive ancillary and medical subspeciality expertise and support. Building an advanced head and neck surgical oncology practice with focus on education and research outside of this environment requires strategic organization, education, and team building endeavors to maintain the highest quality of clinical care and program development. Our success in building and growing this quaternary care practice in a community hospital could serve as a case study in harnessing seasoned labor pools, energetic and experienced educators, and limited resources in a strategic and safe fashion.

Learning Objectives:

  1. Understand the clinical limitation and financial restrictions inherent in building and maintaining a high quality head and neck oncology practice outside the auspices of a traditionally academic center.
  2. Incorporate a comprehensive approach to program development including local and regional analysis of resources and experts in the multidisciplinary field, building an education curriculum for support services, including intensivists, medicine, emergency, nursing.
  3. Create a business model to recruit outside revenue sources to support research and quality measure specifically targeted to early patient reported outcomes, and safety, as well as development of head and neck cancer research registry to support longitudinal research.
