Gender Disparities in Academic Otolaryngology by Region According to H-Index

Introduction: This study aims to investigate sex disparities in academic otolaryngology related to faculty rank and h-index across various geographic regions in the United States. H-index serves to quantify an individual’s scholarly output over time.

Methods: Academic rank was recorded for individual faculty members from every available academic otolaryngology department across the U.S. and its territories. H-indices for each physician were obtained using the Scopus database. All included data was obtained in 2022. Faculty members for which rank could not be obtained were excluded. Prior data published in 2013 was used for comparison. Geographic regions were defined as Southeast (SE), Southwest (SW), Northeast (NE), West (W), Pacific Northwest (PNW), Midwest (MW), and US territories (UST).

Results: In total, 2,141 otolaryngologists (106 programs) were included, 72.9% male and 21.7% female. Faculty representation in each region was SE (25.4%), SW (1.8%), NE (35.2%), W (13%), PNW (2.8%), MW (21.3%), and UST (0.6%). H-index increased with academic rank for both men and women. Overall, men had higher h-indices compared to women (14.49 vs 8.60, p < 0.001) including at every academic level (assistant professor: 6.53 vs 5.43, p = 0.002; associate professor: 12.61 vs 9.88, p < 0.001; professor: 28.07 vs 22.56, p = 0.003; and chair: 28.10 vs 16.36, p = 0.004). H-indices were further isolated to independent geographic regions across the United States by sex. By region, men had significantly higher mean h-indices than women in the SE (14.37 vs 8.21, p < 0.001), NE (14.36 vs 8.86, p < 0.001), W (10.22 vs 7.41, p = 0.002), and MW (17.5 vs 9.43, p < 0.001) but not in the SW, PNW, or UST.

Conclusions: Gender disparities exist across the US in the field of academic otolaryngology according to rank and research productivity but are not consistent across geographic regions.
