A Multi-center Review of Iatrogenic Subglottic Stenosis Microbiology

Introduction: Preliminary literature suggests a link between airway bacterial colonization and pathologies such as airway stenosis. This study examines microbial communities in iatrogenic subglottic stenosis patients, assessing correlations with disease outcomes.

Methods: This retrospective study analyzed patient records (2018-2023) from three affiliated facilities. Current Procedure Terminology (CPT) coding selected for patients undergoing endoscopic and open airway procedures for stenosis. The most common bacterial growths were identified, and chi-squared analyses compared patient data across culture cohorts. Two-sample t-tests compared the average age and degree of stenosis between cohorts.

Results: Review of 45 patients with 46 tracheal stenosis cultures identified the major pathogenic bacteria as Staphylococcus Aureus (n=14) and Pseudomonas aeruginosa (n=15). Two patients had growth of both. There were 26 males and 19 females, and ages ranged from 24-80. Identified pathogens were not influenced by tracheostomy or smoking status. Patients with P. Aeruginosa were older than S. Aureus patients (51.7 vs. 36.7, p=0.007). Antibiotic-treated patients were more likely to have cultures positive for S. Aureus or P. Aeruginosa (55.5% vs. 22.2%, p=0.026). We next examined the contribution of these pathogens to disease severity. There was no significant difference in the initial average degree of stenosis between S. Aureus and P. Aeruginosa cohorts (68.2% vs 70.5%, p=0.422) or in comparing patients with these bacteria to those without (71.3% vs. 64.5%, p=0.661). There was also no significant difference in the frequency of interventions in patients with these bacteria (3.7 vs. 3.3 per year, p=0.413).

Conclusions: In iatrogenic tracheal stenosis patients, the major pathogenic bacteria were S. Aureus and P. Aeruginosa. Antibiotic treatment of stenosis appears to select for these pathogens. However, their presence did not influence the initial degree of stenosis nor intervention frequency, suggesting a limited impact of these bacteria on disease severity.
