Using Artistic Engagement and Entertainment to Improve the Postoperative Experience

Introduction: Hospitals often lack formalized initiatives dedicated to improving the hospital experience for adult patients, a contrast to pediatric patient engagement programs facilitated by child-life specialist services. This study aims to assess the impact of creative engagement and entertainment activities on adult patients undergoing surgery.

Methods: In this prospective, pilot randomized trial, adult patients 18 years or older undergoing postoperative care were recruited from the otolaryngology-head and neck surgery service between January and July 2023. Patients with hospital stays over 15 days were excluded. Patients were randomly assigned to a control or intervention cohort. The intervention group received entertainment packets containing adult coloring pages, art/photography magazines, and puzzles (jigsaw, word puzzles, and Sudoku). Both groups completed a 9-question survey on their satisfaction with inpatient experience. Data analysis was conducted using the R statistical package.

Results: The final study population consisted of 17 patients in the intervention cohort (mean age 58 years, 76% male, 82% white, 47% college graduates) and 17 patients in the control cohort (mean age 67 years, 41% male, 65% white, 35% college graduates). The average length of stay was 4.6 days for the intervention group and 5.3 days for the control group (p = 0.543). Significantly, 71% of intervention group patients reported a positive postoperative experience, compared to 29% in the control group (p = 0.016). Similarly, 82% of intervention group patients found their hospital stay engaging, while only 29% in the control group shared this sentiment (p = 0.005). Moreover, 76% of intervention group patients reported that they were entertained during their hospitalization, compared to 18% in the control group (p = 0.002). Additionally, 94% intervention group patients expressed satisfaction with their care, compared to 71% in the control group (p = 0.175).

Conclusions: This pilot study highlights the positive impact of creative entertainment activities on the inpatient hospital experience for adults undergoing surgery.
