Predictive Patient Factors for Poor Outcomes Following Stapedotomy for Otosclerosis

Introduction: Otosclerosis is a common cause of hearing loss, often treated surgically through a procedure known as stapedotomy. Despite the general success of this procedure, outcomes can vary significantly among patients. Understanding the factors that contribute to these variations is crucial for improving patient care and surgical outcomes. Our goal is to assess patient-related risk factors associated with poor outcomes following stapedotomy for otosclerosis.

Methods: A retrospective chart review encompassing 90 patients diagnosed with otosclerosis and undergoing stapedotomy between 2013 and 2023 was conducted. Demographic information, comorbidities, and smoking history were obtained. Comprehensive preoperative and postoperative audiogram data, including air bone gap (ABG), as well as complications, were collected.

Results: Our cohort comprised of 90 patients, predominantly female (65, 72.2%), with the majority being White (82, 91%), and an average age of 58 years old (SD 14). Among them, 23 patients (25%) reported being current or recent smokers who had quit. The average postoperative ABG across the entire group was 12.25 dB, with one reported intraoperative complication of bleeding. Individuals with a history of otologic procedures exhibited lower ABG gain after surgery compared to their counterparts (3.5 dB vs 9.3 dB, p = 0.312) and higher overall postoperative ABG (16.3 dB vs 13.2 dB, p = 0.597). Smokers reported lower postoperative ABG gain compared to the non-smoker category (8.6 dB vs 12.3 dB, p < 0.05). Additionally, individuals with comorbidities had a lower postoperative ABG gain (1.2 dB vs 7.2 dB, p = 0.301) and a higher postoperative ABG gap (14.5 dB vs 13.4 dB, p = 0.801) compared to those without comorbidities.

Conclusions: Smoking status emerged as a significant independent risk factor impacting hearing outcomes within our patient cohort. While a history of otologic procedures and ongoing comorbidities also showed effects on the outcomes of stapedotomy procedures, they were not statistically significant.
