National Benchmarks for Operative Time and Hospital Length of Stay in Parotidectomies

Introduction: As the healthcare system in the US continues to experience high volumes, it is crucial to understand surgical factors that may influence operating room or hospital bed utilization after parotidectomy. Defining these benchmarks can set important national quality metrics and affect insurance reimbursement.

Methods: This is a retrospective cross-sectional study of all adults who underwent parotidectomy for a primary parotid neoplasm between January 2011 and December 2021 using the American College of Surgeons, National Surgical Quality Improvement Program (ACS-NSQIP) database. Surgical procedures for superficial and total parotidectomies with or without facial nerve dissection or resection were identified using CPT codes. Prolonged operative time (longer than 194 minutes) and prolonged length of stay (LOS, more than 1 day) were defined as above the 75th percentile. Demographic data was compared between groups using a Chi-squared test and operative time and LOS were compared using Kruskal-Wallis tests. Multivariable logistic regression was used to identify surgical risk factors that predict prolonged operative time or LOS.

Results: Prolonged operative time was independently associated with malignant tumors (aOR 2.7, 95%CI: 2.4-3.0), total parotidectomy with facial nerve sacrifice (aOR 2.3, 95% CI 1.7-3.0), and simultaneous procedures including fat/nerve/synthetic grafts (aOR 3.6, 95% CI 3.0-4.5), local/pedicled flap reconstruction (aOR 1.4, 95%CI 1.2-1.65), neck dissection (aOR 3.0, 95%CI 2.5-3.6), and free flap reconstruction (aOR 67.7, 95% CI 21.4-214.5) (p < 0.001 for all). Similarly, these features were all independently associated with prolonged LOS (p < 0.001 for all).

Conclusions: Malignant tumors, greater extent of parotidectomy, and simultaneous procedures were each independently associated with prolonged operative time and LOS. These findings will inform operating room and hospital bed resource assignment and should be considered when determining insurance reimbursement and bundling rates for combined simultaneous procedures.
