The Impact of Patient Specific Implants (PSI) on Treatment of Complex Facial Injuries in Ukraine

Introduction: In this presentation, we will discuss how the use of patient's specific implants (PSI's) has played a major role in all aspects of multiple missions in war-torn Ukraine. As expected, the facial deformities produced by war injuries have been extraordinary and complex. The use of PSIs has allowed for the management of a higher volume of cases and likely provided more optimal care than could have taken place without them. Preoperative planning sessions are more involved with such PSI use but have produced improved preparations for such cases. We will review the process for implant production using case examples.

Methods: This was a case series of PSI placement with CT review.

Results: In total, 38 PSIs were placed during three mission trips. There were 14 orbital and 24 mandible, midface, or upper face. The 13 mandibular implants were used for fibula fixation and 11 for mandibular defect reconstruction without fibular flap. Only 1 mandibula implant was removed after placement. The follow up CT scan in 34 cases showed proper position of implant in 6-12 months.

Conclusions: The PSI is the modern technology that allow precise planning online in advance. From the three successful face to face mission trips the total 38 PSI was placed for facial reconstructions. Overall, the number is over 500 produced by volunteers of principally Ukrainian bioengineers.
