Laryngotracheal Reconstruction (LTR) Outcomes in Pediatric Tracheostomy Patients

Introduction: Pediatric tracheostomy patients contribute to a population that may require laryngotracheal reconstruction (LTR) to facilitate decannulation. Depending on patient history and clinical factors, a double stage procedure (ds) maybe necessary vs a single stage (ss). We sought to identify factors that impact decannulation process following LTR.

Methods: Comprehensive chart review utilizing billing codes from January 1, 2005 through April 30, 2023 identified pediatric tracheostomy patients who underwent LTR at a tertiary pediatric hospital. Data abstracted included demographics, socioeconomic factors, and clinical characteristics. Multivariate regression and survival analysis were used to identify factors associated with successful decannulation following LTR.

Results: Of the 89 patients identified, 56 (63%) underwent dsLTR and 33 (37%) had ssLTR. Time to decannulation from LTR ranged from 0.4 months to 1.44 years with median of 6.2 months. Number of and type of procedures (endoscopic), and time to last follow-up where all significantly higher in the ds cohort vs the ss. In the ds cohort, race, prematurity, and number of procedures were significantly associated with a longer time to decannulation on multivariate analysis. In contrast, higher birth weight, fewer procedures and endoscopic procedures where significantly associated with faster decannulation on univariate analysis.

Conclusions: Open and endoscopic airway procedures address airway obstruction to facilitate safe decannulation. Patients requiring more procedures and those with open procedures had a longer decannulation time.
