Effectiveness of Balloon Sinuplasty for Pediatric Chronic Rhinosinusitis

Introduction: Balloon Sinuplasty (BS) offers a minimally invasive alternative to Functional Endoscopic Sinus Surgery (FESS) for treatment of pediatric chronic rhinosinusitis without nasal polyposis (pCRSsNP). The purpose of this study is to evaluate the effectiveness of BS in the treatment of patients with pCRSsNP.

Methods: A retrospective chart review was performed of children under 18 years with pCRSsNP that had BS between 2018 and 2023. Of 183 screened patients, 95 met inclusion criteria (no history of BS, past/concurrent FESS, concurrent turbinate reduction, cystic fibrosis). Demographics, comorbidities, and pre and postoperative symptoms were recorded. Outcomes were episodes of recurrent sinusitis (RS) within 1 year, and repeat sinus surgery post-BS.

Results: Data from 95 patients with documented follow up (58 male (61.1%), 37 female (38.9%), mean age 9.4 ± 4.1 years) was analyzed. All 95 had undergone maxillary BS, 23 underwent concurrent frontal BS (24.2%), 3 underwent concurrent sphenoid BS (3.2%), and 1 underwent concurrent frontal and sphenoid BS (1.1%). No complications were reported. Symptom improvement (SI) was reported by 76 patients (80%) at their 1 month follow up. Another 46 patients (48.4%) reported SI with no RS within 1 year. Additionally, 43 patients (45.3%) reported at least 1 episode of RS post-BS. Only 6 patients (6.3%) reported no SI, but did not report RS within 1 year post-BS. Few patients (4, 4.2%) underwent additional sinus surgery post-BS. Among patients with allergies, 17/44 (38.6%) and 13/26 (50.0%) patients with GERD reported SI post-BS. Immunoglobulin (IG) deficiency was present in 14 (14.7%) patients, and only 3 (21.4%) reported SI post-BS. Logistic regression analysis revealed that patients with concurrent frontal BS were less likely to experience SI (OR = 0.1, 95% CI = 0-0.8, p = 0.03), and patients with IG deficiency were more likely to develop RS within 1 year post-BS (OR=4.1, 95% CI=1-16.6, p = 0.047).

Conclusions: Children with multi-sinus BS and comorbidities have poorer outcomes after BS; more research is needed to determine the best candidates for BS.
