PROGRAM DESCRIPTION: Laryngo-Pharyngeal Reflux Disorders (LPR) is a prevalent condition characterized by the backflow of stomach acid into the larynx and pharynx, causing symptoms like throat clearing, hoarseness, coughing, and a lump sensation. LPR presents a significant socioeconomic burden, high medical costs, and potential cancer risk. Unlike typical reflux, it lacks common symptoms like heartburn, making diagnosis challenging. Racial differences may arise from diverse anatomy, culture, diet, and genetics. Recognizing these influences is crucial for effective prevention, diagnosis, and management of LPR.
In Taiwan, LPR is increasingly prevalent. Numerous studies utilized various examinations such as saliva pepsin tests, laryngoscopy, Dx-pH oropharyngeal probes, manometry, and hypopharyngeal multichannel intraluminal impedance-pH (HMII-pH) monitoring etc. to evaluate LPR. Notable findings include the potential utility of positive pepsin detection in saliva/sputum for predicting better PPI response. Using the sensitive Dx-pH oropharyngeal probe with polysomnography, we noticed that obstructive sleep apnea does not correlate with a higher incidence of LPR episodes. Studies on manometry and impedance-pH monitoring revealed that PPI-unresponsive laryngeal symptoms are linked to psychological comorbidities and sleep disturbance. Composite pH studies highlighted esophageal acid hyposensitivity in isolated LPR patients without concomitant typical reflux symptoms. The composite pH study could also be used to predict Esomeprazole response. The symposium shares Taiwan's experiences in managing LPR in Asian populations, recognizing potential differences from the Western world.

To share Taiwan's expertise in managing LPR in Asian populations, acknowledging potential variations from the Western world.
