PROGRAM DESCRIPTION: Chronic rhinosinusitis (CRS) represents a significant health problem with high worldwide prevalence. CRS with or without nasal polyps often requires frequent hospital visits and medications, and may necessitate one or more endoscopic sinus surgery (ESS). Recurrence after ESS is, however, not uncommon. Failure of ESS and routine postoperative medical treatment could be attributed to different disease, surgeon or patient factors. Patients' quality of life is usually significantly impaired due to long-term nasal obstruction, hyposmia and other chronic sinonasal symptoms. Recalcitrant or difficult to treat CRS implies a set of required diagnostic, medical and possible surgical options to ensure proper disease control and avoid future recurrence.

In a problem-based approach, we will present several recalcitrant CRS case scenarios and real-life application of state-of-the-art diagnostic and treatment strategies. This will include management of type II inflammation with different biologic agents, screening for immunodeficiency disorders, management of residual diseased air cells, the need for wide or extended sinusotomies, middle turbinate management, role of immunotherapy, anti-leukotrienes, long-term macrolides, probiotic treatment as well as new postoperative irrigation options and photodynamic therapy. Our symposium will help the audience develop a check-list for proper evaluation of recalcitrant CRS, as well as for implementing the appropriate medical and surgical approaches in those cases.

Learning Objectives:

Recognize different causes of failure or recurrence after Endoscopic Sinus Surgery for Chronic Rhinosinusitis (CRS).
Apply the appropriate diagnostic procedures for recalcitrant CRS with or without nasal polyps.
Implement new management strategies in cases of difficult-to-treat CRS, including systemic, local and surgical options.
