Introduction: Decision regret (DR) in healthcare is a negative emotional response to a decision that can compromise future patient decision-making. Early identification of factors associated with DR can lead to early intervention from oncology-psychologists and support groups. We aim to identify factors associated with increased DR among participants treated for head and neck cancers (HNC).

Methods: Prospective cohort study of newly diagnosed squamous cell HNC at an academic tertiary care center from 2018 to 2022. QoL, DR, and anxiety were measured using the University of Washington-QoL questionnaire (UW-QoL), Ottawa Hospital Decision Regret Scale (DRS), and visual anxiety scale (VAS). Questionnaires were completed at initial and 12-month visits. Outcomes include factors associated with increased 12-month DR following treatment completion. Mild DR accounts for scores of 1 to 25 and moderate-severe DR corresponds to scores over 25 in the DRS.

Results: Our study included 167 participants, with the majority being male (66.5%), White (92.7%), with a mean age of 62 years. A year (12 months) after treatment, 50.3% had mild DR and 37.7% had moderate-severe DR. Univariate analyses showed patients with no DR had significantly higher average 12-month physical-QoL scores compared to those with moderate-severe DR (15.5 points, 95% CI: -27.2 – -3.8; p = 0.001) and mild DR (9.2 points, 95% CI: -16.8 – -1.6; p = 0.001). Patients with mild DR had a greater improvement in social-QoL at 12 months compared to those with moderate-severe DR (9 points, 95% CI: 2.4 – 15.6; p < 0.001). This trend remained in multivariate analyses, with those that had a decrease in social-QoL at 12 months having higher odds of increased DR (a OR 1.037, 95% CI: 1.001 – 1.075; p = 0.048).

Conclusions: There is an inverse relationship between social-QoL and DR at 12 months post-treatment. Future interventions that target social factors early, such as those delivered via psycho-oncology or support groups, may improve DR. We also discuss additional sociodemographic factors that may moderate these relationships.
