Predicting Hearing Loss Patterns in Children With Suspected Congenital Cytomegalovirus: Insights From Dry Blood Spot

Introduction: Congenital cytomegalovirus (cCMV) infection is a leading non-genetic cause of sensorineural hearing loss (SNHL) in children. While polymerase chain reaction (PCR) analysis of urine and saliva samples is effective within the perinatal period, dry blood spots (DBS) offer a useful, though less sensitive, method of diagnosis during later time periods. This study aims to determine whether the pattern of SNHL can predict DBS positivity in children with suspected cCMV.

Methods: A retrospective chart review was performed on patients who had undergone DBS testing for cCMV at Primary Children’s Hospital in Utah between November 2016 and November 2022. The independent variables were DBS result and reason for testing. The audiometric outcomes measured were type of hearing loss, progression, severity, and laterality. Statistical significance was assessed using Fisher’s exact test and Kruskal-Wallis test.

Results: Data from 72 children revealed 26.4% (n = 19) tested positive for cCMV. Reasons for testing included failed newborn hearing screening (NBHS) or hearing loss (66.7%) and other causes (33.3%). Among the 19 children who tested positive, 12 were found to have hearing loss with 100% (n = 12) with asymmetric hearing loss compared to 50% (n = 12) of 24 with hearing loss in the DBS-negative cohort (p = 0.003). Approximately 31.6% (n = 6) of DBS-positive patients developed progressive SNHL compared to 17.0% (n = 9) in the DBS-negative cohort (p = 0.2).

Conclusions: The pattern of SNHL in children is a promising approach to determining the likelihood of cCMV infection. For patients who had hearing loss, DBS positivity is significantly correlated with asymmetric hearing loss when compared to the DBS-negative group. Future research efforts will aim to distinguish and refine the patterns of hearing loss in cCMV infected children compared to those without cCMV infection.
