Hearing Outcomes and Neurological Sequelae Among Children With Congenital Cytomegalovirus Diagnoses

Introduction: This presentation will describe hearing outcomes and neurological sequelae by 6 years of age among US children with a diagnosis of cCMV infection or CMV disease.

Methods: The Audiological and Genetic Database (AudGenDB) is a large multi-institutional database of over 95,000 pediatric patients who have undergone audiometric testing from Children’s Hospital of Philadelphia, Vanderbilt Children’s Hospital, and Boston Children’s Hospital from 2006 to 2019. Children up to 6 years of age with CMV, defined using ICD diagnostic codes for cCMV infection or CMV disease. Hearing loss was defined as pure tone average (PTA) or any frequency threshold ≥25 dB. Long-term neurologic sequelae were assessed using ICD diagnostic codes.

Results: Among 180 children identified with CMV, 75 (42%) had a diagnosis of cCMV infection, 83 (26%) had CMV disease, and 22 (12%) had both diagnoses. The first diagnosis of CMV was recorded within 45 days of life for 30% and after 45 days for 70%; 105 (58%) had no recorded clinical signs within the first 45 days. Of 56 (31%) children with hearing loss, 40 (71%) presented with isolated sensorineural hearing loss. Of those children, 53 had >1 ABR/audiogram, with 41 (77%) children presenting with bilateral hearing loss at their first visit compared to 42 (79%) at their last visit. Including hearing loss, a total of 107 (59%) children exhibited one or more permanent conditions by 6 years. Additionally, 45 (25%) children had a diagnosis of communication delays and 57 (32%) had other developmental issues.

Conclusions: This cohort, characterized by likely delayed diagnoses of cCMV and diverse neurological sequelae, had a substantial proportion of children with isolated hearing loss, communication delays, and other developmental issues. AudGenDB has provided access to longitudinal outcomes for a large and diverse cCMV cohort prior to more recent CMV legislation and state-mandated screenings, and may be useful to inform current efforts in extracting data from electronic medical records for cCMV surveillance.
