Impact of Patient Education on Inpatient Compliance With Pharmacologic Venous Thromboembolism Prophylaxis

Introduction: Venous thromboembolism is a leading cause of preventable death amongst hospitalized patients the risk of which may be greatly reduced by pharmacological prophylaxis in the form of anticoagulation. Unfortunately, rates of patient refusals of medication have been a challenge for providers nationally across specialties.

Methods: We conducted a randomized interventional study from October 2022 to October 2023. Patients undergoing operative intervention with a scheduled inpatient postoperative intervention were included in the study. Patients in the intervention groups received a digital or print copy of a patient education flyer informing the patient of the risk of VTE and associated prevention strategies. Verbal education by the nurse navigator regarding VTE was performed guided by the accompanying flyer. Control patients received the typical preoperative education protocols. VTE prophylaxis prescribed, rates of refusals, length of stay and VTE status at discharge were recorded.

Results: A total of 19 patients were included within each arm of the study (educational intervention and control). There was no difference in the pharmacologic prophylaxis prescribed across clinical groups or length of stay. The rate of VTE pharmacologic prophylaxis refusal was 6% in the study intervention groups as compared to 41% in the control group (p < 0.05). There were no observed VTE in either patient cohorts.

Conclusions: A dedicated patient education protocol encorporating both verbal and visual education (patient education flyer) may positively impact rates of compliance amongst inpatients receiving pharmacological prophylaxis for VTE. This could be impactful for decreasing rates of VTE overall in hospitalized patients across specialties.
