PROGRAM DESCRIPTION: Children with Down Syndrome and other craniofacial and neuromuscular disorders are at higher risk for obstructive sleep apnea (OSA) and residual OSA after adenotonsillectomy than those without. In early 2023, the FDA approved the hypoglossal nerve stimulator for use in children with Down Syndrome over age 12, and the device is becoming more widely available to patients and surgeons. In this session, pediatric otolaryngologists and sleep medicine experts from diverse geographic locations will share their experience with upper airway simulation in children with Down Syndrome and other conditions. Panelists will debate the pros and cons of hypoglossal nerve stimulation in children. Cases will highlight clinical controversies including common complications, the role of obesity in success, and concerns unique to a population with a high prevalence of neurosensory processing disorders. Finally, attendees will hear from a third panelist who will review the potential role of hypoglossal nerve stimulation in pediatrics beyond Down Syndrome.

Learning Objectives:

  1. Counsel families on the indications, implant process, and potential for successful treatment of OSA with hypoglossal nerve stimulation in children with Down Syndrome.
  2. Discuss the challenges and limitations of hypoglossal nerve stimulation for treatment of OSA in children with Down Syndrome.
  3. Understand the potential utility and limitations of hypoglossal nerve stimulation for the wider population of children with challenging OSA beyond Down Syndrome.
